Why the Neck Takes the Worst of a Collision

The adult head weighs ten to twelve pounds. It balances on seven small vertebrae held in position by muscles and ligaments that evolved for looking around, not for absorbing crash forces. When your car is struck, the seat pushes your torso forward before your head has begun to move. For a fraction of a second the neck is left behind, and it is that lag that does the damage.

The S-curve moment

High-speed imaging of volunteers in rear-end impacts revealed something the old model missed. Researchers had assumed the neck simply snapped backward and then forward. What actually happens first is that the lower neck extends while the upper neck is still flexed, bending the spine into a brief S shape that no normal movement produces.

This matters because it happens in roughly the first hundred milliseconds, well before your muscles can react. Bracing does not help. The tissue is loaded past its normal range while you are still processing that you have been hit.

What gets injured

The facet joints are the paired joints at the back of each spinal segment that guide and limit motion. They are richly supplied with nerve endings, and they are compressed and sheared during the S-curve phase. Facet injury is one of the best documented sources of persistent neck pain after a collision, and it is a common reason pain is felt more toward the back and side of the neck than the front.

The anterior longitudinal ligament runs down the front of the spine and resists backward bending. Simulation work has shown this ligament stretching beyond its normal limit during whiplash loading. Ligaments do not have the blood supply that muscle does, so they heal slowly and often with less organized tissue than they started with.

The discs sit between the vertebral bodies and take shear loading poorly. Prospective imaging studies following whiplash patients have documented disc changes appearing after collision that were not present before.

Why symptoms are delayed

Torn tissue does not hurt immediately. The inflammatory response that produces swelling, stiffness, and pain takes hours to build and typically peaks somewhere between one and three days out. This is the single most common reason people skip evaluation after a crash. They feel shaken but not injured, they go home, and they wake up on day two barely able to turn their head to check a blind spot.

Headaches that start at the base of the skull, pain between the shoulder blades, jaw tightness, and difficulty concentrating all trace back to the same structures and all commonly follow the same delayed pattern.

What we do about it

We examine segment by segment to find which levels have lost motion and which are moving too much, then treat both. Restoring joint motion early, while the tissue is still remodeling, gives the neck a better chance of healing in a functional position rather than a guarded one. We document findings thoroughly, which matters if there is a claim involved.
